I was in A&E Wednesday night the patient I was with only took 4 hours for about 15mins value added.

Utter shit hole as well.

Over run with 'tourists'

Oh dear. Did they have to wait while others were treated? "Tourists", eh? Shithole, eh? Say what you mean. Trust the Tories with the NHS, eh?

Been in HRI 3 times this week.

Positives

Nice new cubicles in A&E ambulance admissions area.

Negatives

Fuck me same old waste

1) Numerous relatives, myself included, turning up to the wrong desk having to be re-directed to a different reception area in my case even after receiving so called directions - we're either all thick or just possibly the instructions / floor directions and signage are very misleading. I wonder f they will tell 100, 1000, or 10000 before someone thinks about the root cause

2) Father seen by about 4 different people with no obvious continuity between them, myself and my sister having to answer all te same questions 2 & 3 times over

3) Statement /promises made my dad around sorting out specific issues made and then totally not acted upon

4) The 'old peoples team' presumably their role to help assure and provide info to older patients, go home at 5pm.

5) The toliet on a 5 bed ward out of order now for 3 days (& counting) , so rather than my dad managing to hobble through himself he has to go for an assisted trip down the corridor

6) The place awash with student nurses who in some cases appeared to be struggling a bit but not getting the direction they needed

This list is endless


Yep keep throwing money at it 

Fuck the short term we will build a 1000 year Hull City Reich

The A and E we were in last night had had a 'Rapid Improvement Event' done to it some months earlier. I think it is their way of faking a Kaizen Blitz event but it was like NOTHING I would recognise but then I was taught by the originators of such methods so a remain puzzled.

Meanwhile....

1) No patient flow, no division of labour, 'craft' processes everywhere with each person doing it different.
2) delays and waiting between every step of the 'process' requiring calling out of names, waiting, and all sorts of finding people and information related 'work'. That is PURE waste. 30% of names called got zero response - gone home? Dead in the street?
3) hand washing station not used by leaving or arriving staff.
4) no visible management, Key performance indicators on management board > 2 months out of date
5) filthy waiting area
6) water station no cups
7) vending machines 2 of 3 'out-of-order.
8) when arrivals slowed admin staff sitting around doing nothing even though their work area was a mess
9) little clock showing when he next toilet clean should have been done was over one hour overdue
10) loads of wandering backwards and forwards by staff which seemed to be circa 30% of their time.

Don't need to be FW Taylor or WE Deming to see it was overstuffed and under managed with no modern operations management principles in place whatsoever.

Patient journey and flow efficiency was about 2% - no fucking wonder it costs so much and drains money and morale like a black hole.

Needs knocking down and starting again. - new paint and mental health friendly cubicles though.

Literally crawling over millions in waste to get a little work done.

I spend a significant amount of time in HRI A&E, both majors and minors. More TBF in the latter.

Whatever the time or demand (number of people waiting) you'll be waiting 4 hours to be treated. I've seen the place both almost completely empty and heaving full. It seems the same regardless.

I've had limited medical training, however, what I've seen is staff, generally do anything but pay lip service to the initial problem/complaint. I know they'll probably be covering their backsides from "Injury Lawyers 4U". It appears, from what I've seen, that no one wants to make a decision to send people away for simple to treat ailments.

I think two things are prevalent with the staff. A fear of making a mistake due to the harsh consequences of doing so - being sued/disciplined - so they've become risk averse to the extreme. And they are not empowered to lead from the front -because bad leaders don't make bad decisions, bad leaders do not make a decision at all...

If you went to see a GP with some of the ailments I've seen in HRI, you'd be out the door in 5-10 minutes after a quick consultation. Perhaps putting them in there will speed things up.

Perhaps putting them in there will speed things up.

There aren't enough to man the surgeries.

Genuine question - have you directed your fair complaints to a manager of some sort?

Fair point. IMO it's probably time to look at whether Drs surgeries are the best way/place to consult as a first line nowadays.

If I'm at work and need to go to the Docs with a minor problem/ailment, I've got to drive/walk/get a taxi 15 miles to my GP. I drive past numerous NHS premises en route. It'd be quicker easier if I could pop in, get seen and if necessary a prescription for what it is I need (I rarely go to the Drs BTW). Most enquiries don't need repeat visits and if they do you could create an enquiry team at your nearest/most suitable location.
